1. Owner acknowledges the risks associated with boarding a pet. Owner assumes all risk of illness, disease, or injury to owner’s pet and assumes all liability for any injury to your pet or for damage pet causes while staying at Oswego Veterinary Hospital (OVH). *

2. OVH provides bedding, bowls, and leashes and does not accept those itmes with a boarding pet unless a bedding waiver has also been signed by the owner. *

3. Owner authorizes OVH to do whatever is necessary in case of pet’s illness or emergency when owner cannot be contacted. This may include diagnostics, treatment, or medication. *

4. If owner does not provide verification of current vaccines or fecal test, owner authorizes OVH to update these requirements at owner’s expense. *

5. Owner certifies that pet has not been exposed to any communicable disease for 30 days prior to pet’s arrival. Owner recognizes that OVH reserves the right to decline boarding to any pet that is noticeably ill. *

6. Owner certifies that pet has been treated with a flea preventative within the last 30 days. All pets will be checked for fleas and ticks upon arrival and treated if necessary at owner’s expense. *

7. Owner agrees to pay for all services provided to pet and agrees to the daily boarding rate that is in effect on the day pet is checked into boarding. Estimate for stay and extra services is available upon request. Owner further agreees that pet shall not leave facility until all boarding charges are paid in full. *

8. Owner agrees to pay increased rates for the time periods surrounding the following holidays: Christmas Day, New Years Day, Spring Break, Easter Day, Memorial Day, the Fourth of July, Labor Day, and Thanksgiving Day. *

9. Owner acknowledges that pets can only be checked in and out of boarding during open business hours (Monday – Saturday). Owner acknowledges that if pet is picked up after 12pm, owner will incur a charge for an additional full day of boarding. *

10. To confirm reservation, owner will provide a current credit card number at the time of booking. Owner recognizes that if owner cancels reservation with less than two business days notice, the card will be charged for a day of boarding for reach reserved space. *

11. Owner agrees that signature and authorization below applies to this boarding reservation and any future reservations. This also will apply for all existing pets and any future pets of owner that may utilize OVH boarding services. *

Owner understands that a dog may become overly anxious while boarding, and if necessary, a doctor will prescribe a mild sedative or anti-diarrheal for pet’s comfort. In this situation, OVH will make every effort to contact owner prior to any such medication administration.
If walks or playtime are included in dog’s extra services, owner recognizes and accepts the potential risks involved in such an activity. Owner assumes the risk for any injury to owner’s dog and assumes liability for injury or for damage dog causes during this type of activity.

Liability Waiver for Boarding Bedding

Oswego Veterinary Hospital no longer accepts bedding for our boarding guests without a signed waiver due to the risk involved with possible ingestion. Your signature below acknowledges that you are aware of the OVH bedding guidelines and the sometimes life threatening risk involved with providing bedding and having requested your pet be provided with fabric bedding in addition to the Kuranda beds we recommend and supply. Your signature below also confirms that you will assume all fiancial responsibility and risk of illness, disease, or injury to your pet from providing such bedding. At this time, we are unable to take dog beds as we are unable to sufficiently launder them. If you would like to provide blankets/towels/t-shirts from home, signing this waiver will authorize those be provided to your pet while here. Our kennel staff will remove any such bedding if they witness destructive behavior by your pet. *
